(Important Reminders)

DO: It is important that you name The ALS Association as the irrevocable owner of the policy and not just the beneficiary if you intend to deduct your premium payments. The IRS does not allow deductions for your premium payments if you retain ownership of the policy.

DO: When naming The ALS Association as the beneficiary of your life insurance policy, be sure to use the Association's full legal
name and tax ID number: The Amyotrophic Lateral Sclerosis Association, ID # 13-3271855. Contact us to obtain the legal name and tax ID number of one of ALSA's Chapters.

WATCH OUT: If you have borrowed against a life insurance policy, a subsequent gift of the policy will create taxable income for you – the difference between the loan balance and the fair market value of the policy.
